The following forms are used in the total and permanent disability discharge process. Each form below can be accessed online and downloaded for printing.

For Borrowers

  • Discharge Application: Total and Permanent Disability (PDF)
    Complete this application to apply for total and permanent disability discharge of a Title IV student loan or TEACH Grant service obligation.

    If you have a Total and permanent disability, this means that:

    1. You are unable to engage in any substantial gainful activity by reason of a medically determinable physical or mental impairment that can be expected to result in death, that has lasted for a continuous period of not less than 60 months, or that can be expected to last for a continuous period of not less than 60 months,

      OR 

       
    2. You are a veteran who has been determined by the VA to be unemployable due to a service-connected disability  
     
 

For Lenders/Loan Holders

 
  • Guaranty Agency Assignment Format
    Guaranty agencies must assign loans to the Department in accordance with the assignment formats that are contained in the GUARANTY AGENCY ASSIGNMENT FORMAT document.

    As always, there are two important exceptions to the guaranty agency assignment format, when applied to TPD discharge assignments:

     

    1. While the loan you assign must have a balance, there is no minimum balance requirement for TPD discharge assignments.

       
    2. If the loan you are assigning is not in default, you should zero fill Tape Positions 427-434 (or leave Box 56 blank on the Assignment Form). (Additional information can be found here.)
